About Your Visit

What should you bring to your appointment:

At Urological Associates of Bridgeport, PC patient care is our number one priority. We will work with you to schedule an appointment with the physician or staff member at the location of your choice.

Please prepare for this by arriving at least 15 minutes prior to your appointment and complete the necessary paperwork prior to your visit (you can also print the forms from this website to allow you plenty of time to accurately fill them out prior to your visit).


You will need to bring

  • A picture ID such as a drivers license or passport
  • Your health insurance card / information
  • A referral, if required by your insurance carrier . If you have a HMO, you must have a referral for each visit
  • Your copay / deductible required by your insurance carrier
  • The name and number of your referring physician and / or your primary care physician
  • A copy of your medial records and related imaging studies relevant to your current health care concern (from your primary care physician or referring physician)
  • A list of all medications you are taking, including the dose
  • A list of your questions /concerns
  • A friend or family member to take notes and to help with questions
  • Completed Patient Intake form and other pertinent forms

Please read Before Your Visit: Welcome Letter For Our Patients before your first visit to our office. It provides important information about our office policies and appointment procedures. In addition, we encourage you to print out the each of the forms below, fill them out in the comfort of your own home or office and then simply bring them with you to your visit.

For a copy of the Health Insurance Portability and Accountability Act – HIPAA (rev 2013), please request a copy from our offices.

 


Patient Portal

The Urological Associates Patient Portal. Our Patient Portal offers patients the ability to:

  • Update your medical information
  • Send us refill requests
  • Send us appointment requests
  • Send a message to your doctor and staff

 


Insurance Information

Our physicians and staff are participating providers for most major insurance programs. However, please check your insurance providers list of participating physicians to be sure your program allows you to be seen at our office. Also, please review with your insurance provider to clearly understand the rules and limitations on your coverage and reimbursement.

Please keep in mind that your selection of an insurance provider is a direct contract between you, your employer and the insurance provider. Final decisions for coverage and reimbursement rest with your insurance provider.

Urological Associates of Bridgeport is pleased to announce the arrival of our quick pay portal, www.quickpayportal.com. This function will allow you to securely and conveniently pay your bill online with a credit card simply by entering your patient invoice statement ID and selecting sign in. Your payments will be posted immediately and we accept Visa, MasterCard, Discover and American Express for your convenience.
